Give each reviewer a fresh, empty, mode-700, backdated directory as its TMPDIR, named in the reviewer prompt as the only place temporary files may go. After the round, an entry in it or a changed modification time is a broken round; this catches a file written and deleted again. State the check's limit: a reviewer that ignores TMPDIR, writes elsewhere and deletes is caught only by its own report.

Replace the single in-session paad:pushback and paad:alignment runs with three stages of rounds (spec pushback, plan pushback, alignment), each round a fresh review-only subagent, following mode 1 of docs/paad-review-doctrine.md by link. Add the round procedure, the spec and plan header rule, trusted-author resume, round comments without marker lines, and the planning-issue links. Update the skills index pipeline wording.
